Experienced and compassionate nursing professionals are needed to provide skilled care to patients in their homes. In this dynamic, field-based role, you will deliver high-quality, patient-centered nursing services while ensuring seamless communication within an interdisciplinary healthcare team. This contract position offers autonomy, a structured schedule, and opportunities for professional growth.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive assessments using the OASIS tool and develop individualized care plans.
• Deliver direct patient care, such as wound management, medication administration, IV therapy, and chronic disease intervention.
• Monitor patient status, identify and report changes promptly, and document all activities in Home Care Home Base (HCHB).
• Educate patients and caregivers on conditions, medication regimens, treatments, and preventive strategies.
• Collaborate with therapists, medical social workers, and aides to ensure coordinated services.
• Participate in staff meetings, case conferences, and pursue continuing education as appropriate.
• Adhere to home health regulatory standards and internal documentation requirements.
• Travel daily within your assigned territory, with mileage reimbursement provided.

Qualifications:

• Active and unrestricted RN license within the state of practice.
• Minimum of 1 year of recent field-based Home Health RN experience.
• Proficient in Home Care Home Base (HCHB) and familiar with OASIS assessments.
• Reliable transportation, valid drivers license, and up-to-date auto insurance.
• Demonstrated ability to work independently, prioritize effectively, and maintain high standards of patient care and documentation.
• Strong communication, critical thinking, and organizational skills.
• BSN and experience handling a caseload of 2532 points per week preferred.
• Understanding of Medicare and home health compliance standards is an asset.

$2,011 - $2,127/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
