Deliver individualized, evidence-based care as a Home Health Registered Nurse, making a difference in patients lives within the comfort of their own homes. This full-time, 13-week contract opportunity places you at the heart of care coordination, patient education, and clinical excellence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based patient care (35 patient visits per day, managing a caseload of 2532 points weekly)

Key Responsibilities:

• Conduct comprehensive in-home nursing assessments using OASIS and develop tailored care plans.
• Deliver skilled interventions: medication administration, wound care, IV therapy, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and promptly communicate updates to the interdisciplinary team and physicians.
• Offer patient and family education regarding ongoing care, medications, and preventive strategies.
• Ensure thorough, accurate documentation via Home Care Home Base (HCHB).
• Collaborate with a team of therapists, social workers, and home health aides to ensure quality, coordinated care.
• Maintain agency and regulatory standards for documentation and service delivery.

Qualifications:

• Active, unencumbered RN license in the state of practice
• Minimum of 1 year recent experience in Home Health (field experience highly desired)
• Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS assessments
• Reliable transportation, valid drivers license, and proof of auto insurance
• Strong clinical assessment, time management, and communication skills
• Ability to work independently and adapt to a flexible schedule
• BSN and prior experience managing a 2532 point caseload preferred
• Knowledge of Medicare and home health standards is an advantage

$2,011 - $2,127/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
