Essential Qualifications:

• Active RN license in good standing
• Minimum of two years recent home health nursing experience
• Experience managing or supervising a clinical team preferred
• Demonstrated knowledge of home health regulations and best practices
• Strong communication, organization, and problem-solving abilities
• Ability to coordinate multidisciplinary care and ensure compliance with established standards

Primary Responsibilities:

• Manage home health clinical operations, ensuring consistent delivery of high-quality, patient-centered care
• Supervise and mentor field nursing staff, providing guidance and clinical oversight
• Facilitate orientation and ongoing education for clinical team members
• Conduct regular case reviews, oversee care plans, and monitor performance outcomes
• Collaborate with physicians, therapists, and other healthcare professionals to optimize patient results
• Maintain accurate documentation and adherence to all regulatory requirements
• Drive continuous quality improvement initiatives within the department
