Job Description

An exciting opportunity is available for a compassionate and skilled Registered Nurse with home health experience. In this important field-based role, youll deliver high-quality, patient-centered nursing care directly in patients homes. The focus is on promoting positive outcomes, ensuring continuity, and empowering patients and families through education and coordinated care.

Schedule & Contract Details:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week (typically 35 home visits per day)
  • Daily travel within an assigned territory (mileage reimbursement included)
  • Electronic Documentation: Home Care Home Base (HCHB)
  • OASIS assessment tool used for care planning

Key Duties:

  • Complete OASIS assessments and establish tailored care plans with each patient.
  • Deliver skilled clinical care such as wound management, IV therapy, medication administration, and chronic disease oversight.
  • Monitor patient progress, promptly communicate changes, and advocate on behalf of patient needs.
  • Educate patients and families on treatment protocols, medication regimens, and healthy living strategies.
  • Maintain meticulous electronic documentation and uphold state, federal, and agency regulatory standards.
  • Work collaboratively with physical and occupational therapists, medical social workers, and home health aides.
  • Participate in interdisciplinary case conferences, staff meetings, and ongoing training.

Required Qualifications:

  • Active RN license, in good standing
  • At least 1 year of recent home health field nursing experience
  • Proficiency with Home Care Home Base (HCHB) and OASIS required
  • Strong assessment, communication, and organizational abilities
  • Reliable car, valid drivers license, and auto insurance
  • Independent work ethic and adaptability to a mobile work environment

Preferred:

  • Bachelors degree in nursing (BSN)
  • Familiarity managing 2532 point caseloads
  • Understanding of Medicare and home health compliance
