Take your nursing expertise into the community setting as a Home Health Registered Nurse. This contract position focuses on delivering skilled, patient-centered care directly to individuals in their homes, ensuring continuity and excellence throughout the care journey. Youll work independently, yet as part of a larger interdisciplinary team, utilizing your critical thinking and organizational skills to manage a designated caseload and promote the best possible outcomes.
Schedule & Setting:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Field-based (providing care in patient homes)
- Daily travel within assigned territory (mileage reimbursement provided)
- Caseload expectation: 2532 points per week (averaging 35 patient visits per day)
- Utilizes Home Care Home Base (HCHB) for electronic documentation and OASIS as the assessment tool
Key Responsibilities:
- Perform thorough OASIS assessments and design individualized care plans
- Provide a range of skilled nursing services, such as wound care, IV therapy, medication management, and chronic disease oversight
- Regularly evaluate patient status, communicate changes promptly, and collaborate with physicians and other home health professionals
- Educate patients and families on diagnoses, medications, and overall care plans
- Accurately document care activities in HCHB and participate in ongoing case conferences and educational meetings
- Adhere closely to agency and regulatory standards
Qualifications:
- Active RN license in good standing (state-specific)
- At least 1 year of recent home health field experience
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Strong clinical assessment, communication, and organization skills
- Reliable transportation, valid drivers license, and auto insurance
- BSN and experience with caseload management are preferred
