An excellent opportunity awaits for an experienced and dedicated Registered Nurse (RN) specializing in Home Health nursing. In this contract position, youll deliver skilled nursing services to patients within their homes, ensuring individualized, high-quality care while supporting their recovery and wellness goals.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop tailored care plans
• Provide direct skilled nursing care such as medication administration, wound care, IV therapy, and chronic disease management
• Monitor patient progress, report changes, and coordinate closely with physicians and care teams
• Offer patient and family education regarding disease management, medication usage, and prevention strategies
• Maintain timely and accurate electronic documentation in Home Care Home Base (HCHB)
• Collaborate with interdisciplinary teams, including therapists, social workers, and aides, for seamless care coordination
• Attend case conferences, staff meetings, and participate in continuing education opportunities
• Ensure compliance with all regulatory and agency care standards

Qualifications:

• Active RN license in good standing
• At least 1 year of recent Home Health field experience required
• Proficiency in OASIS and Home Care Home Base (HCHB) essential
• Reliable transportation, a valid drivers license, and auto insurance required
• Strong clinical assessment, organizational, and communication skills
• Ability to operate independently and manage a flexible, field-based caseload
• BSN and prior experience managing 2532 points per week preferred
• Knowledge of Medicare and home health compliance standards is a plus

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
