Job Description

Provide skilled nursing care to patients in a home setting as an experienced Home Health Registered Nurse (RN). This role requires clinical expertise, strong organizational skills, and the ability to work independently while delivering patient-centered care. Your responsibilities include performing comprehensive OASIS assessments, developing and managing individualized care plans, and collaborating with an interdisciplinary healthcare team to support positive patient outcomes. Patient and family education, careful monitoring of ongoing progress, and adherence to regulatory standards are essential components of this position.

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop tailored care plans.
  • Provide skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress and promptly report any changes in condition to care team members and physicians.
  • Educate patients and families on health conditions, treatments, medications, and preventive care measures.
  • Maintain accurate and timely documentation using the Home Care Home Base (HCHB) electronic system.
  • Collaborate with therapists, social workers, and home health aides to ensure coordinated care delivery.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Comply with all regulatory and agency standards regarding documentation and patient care.

Qualifications:

  • Active RN license in the state of practice, in good standing.
  • Minimum of 1 year recent home health experience, preferably field-based.
  • Proficiency with Home Care Home Base (HCHB) and OASIS required.
  • Strong clinical assessment and critical thinking abilities.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently managing a flexible field schedule.
  • Excellent communication, time management, and organizational skills. Preferred:
  • Bachelor of Science in Nursing (BSN).
  • Experience managing 2532 points per week.
  • Knowledge of Medicare and home health compliance standards.

Schedule & Location:

  • Full-time, 13-week contract.
  • Monday through Friday, 8:00 AM 5:00 PM.
  • Field-based with daily travel within assigned territory; mileage reimbursement provided.
  • Caseload typically includes 35 patient visits per day.
