An excellent opportunity is available for an experienced and compassionate Registered Nurse with a background in home health. This full-time, 13-week contract focuses on delivering high-quality, patient-centered nursing care to individuals in their homes. The role is best suited for RNs who are self-directed, organized, and adept at managing multiple case assignments independently.

Schedule & Details:

• Monday Friday, 8:00 AM 5:00 PM
• Expect 2532 points per week (typically 35 patient visits daily)
• Field-based, providing care in patient homes
• Daily travel within assigned territory (mileage reimbursement available)

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions: wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress and promptly report changes to physicians and the care team
• Educate patients and families about disease management, medication regimens, and preventative health
• Maintain accurate, timely documentation in Home Care Home Base (HCHB)
• Collaborate with an interdisciplinary team for coordinated, holistic care
• Participate in case conferences and staff meetings

Qualifications:

• Active RN license in good standing
• At least one year of recent home health nursing experience
• Working knowledge of Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, valid drivers license, and auto insurance
• Strong clinical assessment, organization, and communication skills
• Ability to work independently within a flexible, field-based role

Preferred Experience:

• BSN
• Experience managing a caseload of 2532 points per week
• Knowledge of Medicare guidelines for home health

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
