Job Description

We are seeking an experienced Hospice Registered Nurse (RN) to deliver compassionate, skilled care within home and residential settings. In this role, you will build deep connections with your patients and their loved onesoffering comfort, dignity, and support as part of a collaborative, interdisciplinary team.

Position Details:

  • Schedule: Monday Friday, 8:00 AM 5:00 PM (occasional weekend or on-call rotation may be required)
  • Commitment: 13-week contract, full-time
  • Caseload: Expected 45 patient visits per day
  • Environment: Field-based (includes homes, assisted living, and nursing facilities)
  • Documentation: Utilize Home Care Home Base (HCHB) for all records
  • Travel: Daily travel throughout service area (mileage reimbursement available)

Key Responsibilities:

  • Complete thorough nursing assessments and develop individualized hospice care plans
  • Deliver direct skilled nursing care focused on pain control, symptom management, and overall patient comfort
  • Administer medications and treatments per physician orders and established protocols
  • Monitor changes in patient condition; collaborate with the clinical team for effective care coordination
  • Educate patients, families, and caregivers about disease progression, comfort strategies, and medication management
  • Foster effective interdisciplinary collaboration to address holistic patient needs
  • Ensure accurate and timely electronic documentation in HCHB
  • Uphold compliance with all hospice standards and regulatory requirements

Qualifications:

  • Active RN license in good standing
  • Minimum one year of recent hospice nursing experience
  • Proficiency in Home Care Home Base (HCHB)
  • Strong clinical assessment, communication, and independent time management skills
  • Reliable transportation, valid drivers license, and current auto insurance
  • Compassion, empathy, and emotional resilience essential

Preferred:

  • BSN degree
  • Experience in palliative or end-of-life settings
  • Familiarity with Medicare hospice documentation standards
