Experienced and compassionate registered nurses are invited to work in a dynamic field-based home health environment serving patients in their homes. This contract opportunity offers the chance to deliver one-on-one patient care while maintaining high standards of clinical excellence and patient communication.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Daily travel to patient homes within assigned territory (mileage reimbursement provided)

Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans for each patient.
• Deliver skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient conditions, identify changes, and communicate promptly with the interdisciplinary care team and physicians.
• Provide education to patients and families regarding disease processes, care instructions, medications, and preventive strategies.
• Document all care and patient progress accurately and promptly using Home Care Home Base (HCHB) electronic system.
• Collaborate with therapists, social workers, and other healthcare professionals to coordinate holistic patient care.
• Attend case conferences, staff meetings, and participate in ongoing education.
• Ensure compliance with agency and regulatory standards in patient care and documentation.

Qualifications:

• Active RN license in Washington State, in good standing
• At least 1 year of recent home health (field) experience
• Proficiency with Home Care Home Base and OASIS required
• Strong assessment, critical thinking, time management, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and manage a flexible schedule
• BSN and prior experience with a similar caseload highly preferred
• Familiarity with Medicare and home health compliance a plus

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
