Job Description

Provide compassionate end-of-life care as a Hospice Registered Nurse (RN) in a dynamic, field-based role serving patients in their homes, assisted living, or nursing facilities. This full-time, 13-week contract position is based in Lexington, SC, offering a Monday through Friday schedule from 8:00 AM to 5:00 PM, with occasional weekend or on-call duties.

Key Responsibilities:

  • Conduct thorough nursing assessments and develop individualized hospice care plans.
  • Deliver skilled nursing focused on symptom management, pain control, and patient comfort.
  • Administer medications and treatments following physician orders and hospice protocols.
  • Monitor patient conditions, communicating changes to the hospice medical director and care team promptly.
  • Educate patients, families, and caregivers on disease progression, medication administration, and emotional support.
  • Collaborate with social workers, chaplains, aides, and volunteers to address comprehensive patient needs.
  • Complete accurate documentation using Home Care Home Base (HCHB) in a timely manner.
  • Participate in case conferences, team meetings, and quality improvement efforts.
  • Adhere to hospice policies, procedures, and regulatory guidelines.

Qualifications:

  • Active RN license in good standing in the practicing state.
  • Minimum of 1 year recent hospice nursing experience.
  • Proficiency with Home Care Home Base (HCHB) electronic documentation required.
  • Excellent clinical assessment and communication skills.
  • Strong knowledge of pain management and end-of-life care principles.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently in a field-based environment.
  • Compassionate, emotionally resilient, and patient-focused.

Preferred:

  • Bachelor of Science in Nursing (BSN).
  • Experience in palliative or end-of-life care settings.
  • Familiarity with Medicare hospice regulations and documentation standards.
