Job Description
Provide compassionate care as a Hospice Registered Nurse, supporting patients and families during their end-of-life journey in home and residential environments. This full-time, 13-week contract offers a meaningful opportunity to make a difference through skilled nursing, symptom management, and emotional support.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation may be required
Key Responsibilities:
- Conduct thorough nursing assessments and develop tailored hospice care plans
- Deliver direct skilled nursing care, focusing on patient comfort, symptom control, and pain management
- Administer prescribed medications and interventions according to hospice protocols
- Closely monitor and document changes in patient condition
- Communicate patient updates with the medical director and interdisciplinary care team
- Provide education and support to patients, families, and caregivers on illness progression, medication, and comfort care
- Collaborate with social workers, chaplains, aides, and volunteers to meet holistic needs
- Document care accurately in Home Care Home Base (HCHB)
- Participate in case conferences, team meetings, and quality initiatives
- Ensure care delivery complies with hospice policies and regulations
Required Qualifications:
- Active RN license (state of practice, in good standing)
- Minimum 1 year of recent hospice experience
- Proficiency with Home Care Home Base (HCHB) documentation
- Skilled in clinical assessment and compassionate communication
- Strong background in end-of-life care principles
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to manage an independent, mobile schedule and caseload (45 daily patient visits)
- Emotionally resilient and empathetic
Preferred:
- BSN degree
- Experience in palliative or end-of-life settings
- Familiarity with Medicare hospice guidelines
