Job Description
Deliver high-quality care while making a meaningful impact as a Registered Nurse specializing in home health services. This full-time position offers a 13-week contract providing essential nursing care to patients in their homes. It is ideal for skilled, self-driven RNs who excel in autonomous fieldwork and have a passion for patient-centered care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (approximately 35 patient visits daily)
- Field-based travel within assigned territory (mileage reimbursement included)
Key Responsibilities:
- Complete comprehensive OASIS assessments and formulate individualized care plans
- Deliver skilled nursing interventionswound care, IV therapy, medication administration, and chronic condition management
- Monitor patient status, communicate changes, and collaborate closely with physicians and multidisciplinary teams
- Educate patients and their families regarding disease management, medications, and preventive health practices
- Maintain precise, up-to-date documentation using Home Care Home Base (HCHB)
- Engage in care conferences, staff meetings, and continuing education activities
- Consistently adhere to regulatory guidelines and agency best practices
Qualifications:
- Active, unencumbered RN license in the state of practice
- At least 1 year of recent home health nursing experience
- Proficient in Home Care Home Base (HCHB) and OASIS assessment tools
- Effective clinical assessment, communication, and organizational skills
- Reliable transportation with valid drivers license and current auto insurance
- Ability to manage an independent, flexible field schedule
Preferred:
- Bachelor of Science in Nursing (BSN)
- Prior management of a 2532 point caseload
- Familiarity with Medicare and compliance standards
