Providing compassionate, patient-centered care, this Hospice Registered Nurse (RN) position focuses on delivering exceptional end-of-life support to patients in home or residential settings. This full-time, 13-week contract role involves managing hospice care with empathy and clinical expertise, ensuring comfort and dignity for patients facing terminal illness.
Location:
Positioned in Leominster, MA, this role involves field-based visits to patient homes, assisted living, or nursing facilities within the assigned service area. Leominster offers a tight-knit community environment with convenient access to regional healthcare resources, making it an ideal setting for impactful hospice care.
Responsibilities:
- Conduct thorough nursing assessments and develop individualized hospice care plans.
- Deliver skilled nursing focused on symptom management, pain relief, and patient comfort.
- Administer medications and treatments per physician orders and hospice protocols.
- Monitor patient conditions and communicate updates to the medical director and care team.
- Educate patients, families, and caregivers on disease progression, medication administration, and emotional support.
- Collaborate with interdisciplinary teams including social workers, chaplains, and aides.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB).
- Participate in team meetings, case conferences, and quality improvement initiatives.
- Ensure compliance with hospice policies and regulatory guidelines.
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum 1 year of recent hospice experience required.
- Proficient in Home Care Home Base (HCHB) documentation.
- Strong clinical assessment and communication skills.
- Knowledge of pain management and end-of-life care principles.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to independently manage a field-based schedule with compassion and emotional resilience.
Preferred:
- BSN degree preferred.
- Experience in palliative or end-of-life care settings.
- Familiarity with Medicare hospice regulations and documentation standards.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation may be required
- Caseload of approximately 45 patient visits per day
