Providing compassionate, patient-centered care, this Hospice Registered Nurse (RN) position focuses on delivering exceptional end-of-life support to patients in home or residential settings. This full-time, 13-week contract role involves managing hospice care with empathy and clinical expertise, ensuring comfort and dignity for patients facing terminal illness.

Location:
Positioned in Leominster, MA, this role involves field-based visits to patient homes, assisted living, or nursing facilities within the assigned service area. Leominster offers a tight-knit community environment with convenient access to regional healthcare resources, making it an ideal setting for impactful hospice care.

Responsibilities:

• Conduct thorough nursing assessments and develop individualized hospice care plans.
• Deliver skilled nursing focused on symptom management, pain relief, and patient comfort.
• Administer medications and treatments per physician orders and hospice protocols.
• Monitor patient conditions and communicate updates to the medical director and care team.
• Educate patients, families, and caregivers on disease progression, medication administration, and emotional support.
• Collaborate with interdisciplinary teams including social workers, chaplains, and aides.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Participate in team meetings, case conferences, and quality improvement initiatives.
• Ensure compliance with hospice policies and regulatory guidelines.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum 1 year of recent hospice experience required.
• Proficient in Home Care Home Base (HCHB) documentation.
• Strong clinical assessment and communication skills.
• Knowledge of pain management and end-of-life care principles.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to independently manage a field-based schedule with compassion and emotional resilience.

Preferred:

• BSN degree preferred.
• Experience in palliative or end-of-life care settings.
• Familiarity with Medicare hospice regulations and documentation standards.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Occasional weekend or on-call rotation may be required
• Caseload of approximately 45 patient visits per day
