An experienced Home Health Registered Nurse (RN) role is available in Lenexa, KS, offering a rewarding opportunity to deliver skilled nursing care directly to patients in their homes. This position requires a dedicated, organized professional who excels in independent fieldwork while maintaining high standards of clinical practice and patient communication.
Key responsibilities include conducting comprehensive assessments using OASIS, developing individualized care plans, and providing skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management. Strong coordination with interdisciplinary teams and clear communication with patients, families, and physicians are essential. Accurate, timely documentation in the Home Care Home Base (HCHB) system is required to ensure continuity and compliance with care standards.
Qualifications:
- Active RN license in good standing within the state of practice
- Minimum of 1 year of recent home health experience, preferably field-based
- Proficient with Home Care Home Base (HCHB) and OASIS assessment tools
- Strong clinical assessment and critical thinking abilities
- Reliable transportation, valid drivers license, and auto insurance
- Ability to independently manage a flexible field-based schedule
- Excellent communication, time management, and organizational skills
Preferred:
- Bachelor of Science in Nursing (BSN)
- Prior experience managing 2532 points weekly
- Knowledge of Medicare and home health compliance standards
This full-time, 13-week contract role is scheduled Monday through Friday, 8:00 AM to 5:00 PM, with an expectation of 35 patient visits daily within an assigned territory. Mileage reimbursement is provided.
