An experienced Home Health Registered Nurse (RN) is needed for a full-time, 13-week contract in Lenexa, KS. This role involves delivering skilled nursing care to patients in their homes, ensuring top-quality, patient-centered treatment. The ideal candidate is self-driven, organized, and capable of working independently while upholding clinical excellence and clear communication.
Responsibilities:
- Conduct thorough OASIS assessments and develop individualized care plans.
- Provide skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, recognize changes in condition, and communicate effectively with care teams and physicians.
- Educate patients and families on medical conditions, treatments, medications, and preventive care measures.
- Maintain accurate and timely documentation using the Home Care Home Base (HCHB) electronic system.
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides to coordinate comprehensive care.
- Participate in case conferences, staff meetings, and ongoing education.
- Adhere to regulatory and agency standards for documentation and patient care.
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum 1 year of recent home health experience; field experience preferred.
- Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessments.
- Strong clinical assessment and critical thinking skills.
- Reliable transportation with a valid drivers license and auto insurance.
- Ability to manage independent, flexible, field-based schedules.
- Excellent communication, organizational, and time management capabilities.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience managing caseloads of 2532 points per week.
- Knowledge of Medicare and home health compliance standards.
Contract Details:
- Schedule: Monday Friday, 8:00 AM 5:00 PM.
- Caseload: 2532 points weekly, about 35 patient visits daily.
- Daily travel within assigned territory with mileage reimbursement.
