Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, identify changes in condition, and communicate findings to physicians and care teams
• Educate patients and families regarding disease processes, care plans, and preventive care
• Maintain timely, accurate documentation using Home Care Home Base (HCHB)
• Coordinate with physical therapists, occupational therapists, medical social workers, and home health aides for integrated care delivery
• Attend case conferences, staff meetings, and engage in required continuing education
• Adhere to all regulatory guidelines and standards for documentation and patient care

Qualifications:

• Active RN license (in good standing)
• Minimum 1 year of recent home health experience
• Field experience preferred
• Required proficiency in Home Care Home Base (HCHB) and OASIS assessments
• Strong clinical assessment and critical thinking skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and manage a field-based schedule
• BSN preferred; prior experience with 2532 points per week is a plus
