Job Description
Provide compassionate, expert hospice care as a Registered Nurse (RN) serving patients in their homes or assisted living settings. This full-time, 13-week contract role supports individuals facing terminal illness by delivering personalized care that emphasizes comfort, dignity, and quality of life. The position is based in Leland, NC, with daily travel required within the service area (mileage reimbursement provided).
Key Responsibilities:
- Perform comprehensive nursing assessments and develop individualized hospice care plans.
- Deliver skilled nursing services focused on symptom management and pain control.
- Administer medications and treatments following physician orders and hospice protocols.
- Monitor patient condition changes and communicate promptly with medical directors and care teams.
- Educate patients, families, and caregivers on disease progression, comfort measures, and medication administration.
- Collaborate closely with interdisciplinary team members including social workers, chaplains, aides, and volunteers.
- Complete timely, accurate documentation using Home Care Home Base (HCHB).
- Participate in team meetings, case conferences, and quality improvement initiatives.
- Ensure compliance with hospice policies and regulatory guidelines.
Qualifications and Desired Experience:
- Active RN license in good standing for the state of practice.
- Minimum of 1 year recent hospice nursing experience.
- Proficiency with Home Care Home Base (HCHB) electronic documentation.
- Strong clinical assessment skills and excellent communication.
- Understanding of pain management, end-of-life care, and family support.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently in a field-based role.
- Compassionate and emotionally resilient approach.
- BSN preferred.
- Prior palliative or end-of-life care experience a plus.
- Familiarity with Medicare hospice regulations beneficial.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM.
- Occasional weekend or on-call rotations may be required.
- Caseload of approximately 45 patient visits per day.
