Qualifications & Experience:
- Current RN license in good standing
- Minimum of one year of recent experience in home health as a Registered Nurse
- Proficiency in electronic medical record systems for timely and accurate documentation
- Strong clinical assessment and critical thinking skills
- Ability to independently manage a patient caseload and adapt care plans based on evolving patient needs
- Reliable transportation and the ability to travel to patient residences
- CPR/BLS certification
Key Responsibilities:
- Conduct comprehensive patient assessments in the home setting
- Develop, implement, and revise individualized care plans according to medical orders and patient goals
- Administer medications, perform skilled nursing procedures, and monitor treatment responses
- Educate patients and caregivers on disease management, safety protocols, and wellness strategies
- Collaborate with physicians, therapists, and other healthcare professionals to provide coordinated care
- Maintain accurate patient records and ensure compliance with all regulatory guidelines
- Communicate effectively with patients, families, and team members to foster trust and promote positive outcomes
