Deliver patient-centered care in the comfort of clients homes as an experienced Home Health Registered Nurse. This contract opportunity focuses on delivering skilled nursing services to a diverse patient population, supporting improved outcomes through comprehensive clinical care, strong communication, and seamless interdisciplinary collaboration.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Role Details:

• Full-time, 13-week contract
• Field-based, visiting 35 patients per day within assigned territory
• Manage a caseload of 2532 points per week
• Daily travel required (mileage reimbursement provided)

Primary Responsibilities:

• Complete thorough OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services: wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, recognize changes, and initiate physician/care team communication swiftly.
• Educate patients and families on disease processes, care options, and safe practices.
• Accurately document all care provided using Home Care Home Base (HCHB).
• Coordinate care with physical and occupational therapists, medical social workers, and home health aides.
• Participate in case conferences and team meetings; uphold agency and regulatory standards.

Qualifications:

• Active, unrestricted RN license in the state of practice.
• At least 1 year of recent field-based Home Health RN experience.
• Proficient with Home Care Home Base (HCHB) and OASIS assessments.
• Strong skills in clinical assessment, organization, and time management.
• Valid drivers license, reliable transportation, and auto insurance.
• Ability to work autonomously in a flexible, field-based environment.
• BSN and experience with 2532 point caseloads preferred.
• Familiarity with Medicare and home health regulatory requirements a plus.
