Deliver high-quality, patient-centered care as a Home Health Registered Nurse within an established field-based team. In this 13-week contract role, youll be responsible for providing individualized nursing services and ensuring continuity of care for patients in their homes.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete thorough OASIS assessments and design individualized care plans.
• Deliver skilled nursing including wound care, IV therapy, medication administration, and disease management.
• Monitor patient status, report changes promptly to the interdisciplinary care team and physicians.
• Educate patients and families about care plans, medications, and overall health management.
• Document accurately and efficiently using Home Care Home Base (HCHB).
• Coordinate patient care with therapists, social workers, and home health aides.
• Attend case conferences, team meetings, and required education sessions.
• Maintain compliance with agency guidelines and state/federal regulations.
• Travel daily within the assigned territory; mileage reimbursement provided.

Qualifications:

• Active RN license in good standing for state of practice.
• Minimum 1 year of recent home health field experience.
• Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS documentation.
• Solid clinical assessment, critical thinking, and time management skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to prioritize independently in a flexible, field-based environment.
• BSN preferred.
• Experience managing a caseload of 2532 points per week strongly preferred.
• Familiarity with Medicare and home health compliance is a plus.
