Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete OASIS assessments and create adaptable, individualized care plans for each patient.
- Deliver a range of skilled nursing interventions, from wound care and IV therapy to chronic disease management and medication administration.
- Monitor patient progress, recognize changes, and communicate promptly with physicians and interdisciplinary team members.
- Educate patients and families on diagnoses, care routines, medications, and preventive strategies.
- Document all care activities and assessments accurately in the Home Care Home Base (HCHB) electronic system.
- Work collaboratively with physical and occupational therapists, social workers, and home health aides to ensure holistic patient care.
- Attend case conferences, team meetings, and engage in ongoing professional development.
- Comply with relevant regulatory standards and agency protocols.
Qualifications:
- Active RN license (in good standing)
- Minimum 1 year of recent home health nursing experience; field experience highly preferred
- Strong proficiency in Home Care Home Base (HCHB)
- Prior experience utilizing OASIS assessment tools
- Strong clinical judgment, organizational, communication, and time management skills
- Reliable transportation, valid drivers license, and auto insurance
- BSN and knowledge of Medicare/home health compliance standards preferred
