An opportunity is available for an experienced and compassionate Registered Nurse to deliver high-quality patient care in the home health setting. This contract role offers the chance to work autonomously while making a meaningful difference in patients lives by supporting their recovery and well-being at home.

Schedule & Job Details:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload of 2532 points per week (approximately 35 patient visits per day)
• Field-based: Daily travel to patient homes (mileage reimbursement provided)
• Electronic Documentation: Home Care Home Base (HCHB)
• OASIS assessment tool used

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent home health nursing experience (field experience strongly preferred)
• Proficiency with HCHB and OASIS required
• Reliable transportation, valid drivers license, and auto insurance
• Strong clinical assessment, communication, and organizational skills
• Ability to work independently in a flexible, field-based environment
• BSN preferred
• Experience managing caseloads of 2532 points per week preferred
• Knowledge of Medicare and home health compliance standards valued

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic disease management
• Provide patient and family education on disease processes and care plans
• Collaborate with interdisciplinary teams to ensure coordinated care
• Maintain accurate, timely documentation in HCHB
• Attend case conferences and staff meetings as needed

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
