Providing skilled nursing care in patients homes, this Home Health RN role offers the opportunity to deliver compassionate, patient-centered care while working independently and maintaining clinical excellence. The position is full-time, field-based, and contract, supporting a dynamic caseload within a defined territory.
Location: Lawrenceville, GA
Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
- Evaluate patient progress, identify condition changes, and communicate promptly with physicians and care teams
- Educate patients and families about disease processes, treatment plans, medications, and preventive care
- Maintain accurate and timely electronic documentation using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary healthcare professionals such as physical therapists, occupational therapists, social workers, and home health aides
- Participate in case conferences, staff meetings, and continuing education
- Adhere to regulatory and agency standards for documentation and care delivery
- Manage daily travel to patient homes within assigned territory (mileage reimbursement provided)
Qualifications:
- Active RN license in good standing in the state of practice
- Minimum 1 year of recent home health nursing experience, preferably field-based
- Proficient with Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical assessment and critical thinking abilities
- Reliable transportation with valid drivers license and auto insurance
- Independent, organized, and excellent communicator Preferred:
- BSN degree
- Experience managing 2532 points weekly caseload
- Knowledge of Medicare and home health compliance standards
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM
- 3 to 5 patient visits daily, averaging 2532 points weekly
