Job Description

Deliver high-quality, patient-centered nursing care in a dynamic home health environment as a Registered Nurse. This contract opportunity calls for an experienced and compassionate RN who excels at providing skilled clinical care within patients homes. The position requires expert autonomy, sound judgment, and dedication to ensuring optimal outcomes for patients navigating complex health needs.

Schedule & Assignment Details:

  • Monday Friday, 8:00 AM 5:00 PM
  • 13-week contract commitment
  • Field-based: travel within assigned territory to patient homes (mileage reimbursement provided)
  • Caseload: 2532 points per week (approximately 35 patient visits daily)
  • Documentation via Home Care Home Base (HCHB); OASIS assessments required

Key Responsibilities:

  • Complete OASIS assessments and develop individualized care plans
  • Deliver skilled nursing interventions (wound care, IV therapy, chronic disease management, medication administration)
  • Closely monitor patient progress, identify status changes, and communicate promptly with physicians and interdisciplinary care teams
  • Educate patients and families about treatment plans, medications, disease management, and preventive strategies
  • Maintain timely, accurate documentation using HCHB
  • Collaborate with PTs, OTs, medical social workers, and home health aides for holistic care
  • Participate in case conferences, staff meetings, and professional development sessions
  • Ensure compliance with state, federal, and agency care standards

Qualifications:

  • Active RN license in good standing
  • At least 1 year of recent home health experience; field-based experience preferred
  • Familiarity and proficiency with HCHB and OASIS required
  • Strong clinical assessment, critical thinking, and communication skills
  • Reliable vehicle, valid drivers license, and proof of auto insurance
  • Ability to work independently and manage a flexible travel schedule
  • BSN and knowledge of Medicare/home health compliance preferred
