Deliver skilled and compassionate nursing care where it matters mostdirectly in patients homes. Seeking an experienced Registered Nurse to provide home health services on a full-time, 13-week contract. This role is ideal for someone comfortable working independently, with exceptional clinical, organizational, and communication skills.
Schedule and Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week; typically 35 patient visits per day
- Field-based, with daily travel to patient homes (mileage reimbursement provided)
- Utilize Home Care Home Base (HCHB) for documentation and OASIS for assessments
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop patient-centered care plans
- Deliver skilled nursing services: wound care, medication administration, IV therapy, chronic disease management
- Monitor patient outcomes, coordinate with healthcare team members, and escalate changes in condition to physicians
- Educate patients and their families on health conditions, medications, and self-care
- Ensure accurate and timely electronic documentation
- Collaborate with physical and occupational therapists, social workers, and aides to support seamless, quality care
- Participate in ongoing team meetings and stay current with continuing education requirements
- Adhere to all regulatory and agency standards for clinical and documentation practice
Qualifications:
- Active, unencumbered RN license in the state of practice
- Minimum 1 year of recent home health experience (field preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment
- Strong assessment, clinical judgment, and organizational abilities
- Valid drivers license, reliable vehicle, and auto insurance
- Ability to thrive independently with a flexible field-based schedule
- BSN preferred and prior experience managing similar caseloads a plus
