Seeking an experienced and compassionate Registered Nurse skilled in home health care for an engaging contract opportunity. This position supports patients in their homes, ensuring top-quality, patient-centered nursing within a collaborative and flexible field environment. The ideal candidate is self-motivated, organized, and comfortable providing care independently while maintaining high clinical standards and effective communication.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Responsibilities:

• Complete OASIS assessments and design individualized care plans.
• Deliver skilled nursing services (wound care, medication administration, IV therapy, chronic disease management).
• Monitor patient condition, communicate changes to the interdisciplinary team, and update physicians as needed.
• Offer patient and family education regarding disease management, treatment plans, and preventive care.
• Maintain thorough, accurate electronic records using Home Care Home Base (HCHB).
• Coordinate care with PTs, OTs, social workers, and home health aides.
• Attend case conferences and staff meetings; participate in ongoing education.
• Uphold all agency and regulatory standards for documentation and care delivery.

Qualifications:

• Active RN license (state of practice) in good standing.
• At least 1 year of recent home health experience (field preferred).
• Proficiency in Home Care Home Base (HCHB).
• Demonstrated OASIS experience.
• Strong assessment, critical thinking, communication, and organizational skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to operate independently and manage a field-based schedule.

Preferred:

• BSN degree.
• Experience with 2532 points per week caseload.
• Knowledge of Medicare and home health compliance guidelines.
