Job Description
Deliver exceptional patient care in a rewarding home health setting as a Registered Nurse dedicated to supporting individuals in their own homes. This contract opportunity offers the chance to work independently while leveraging your clinical skills and building strong patient relationships.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans for each patient.
- Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management.
- Evaluate changes in health status, effectively communicate with physicians and healthcare team members, and proactively address patient needs.
- Educate patients and families on diagnoses, medication regimens, and disease prevention strategies, empowering them to achieve optimal health outcomes.
- Document accurately and efficiently using Home Care Home Base (HCHB).
- Coordinate care with physical therapists, occupational therapists, medical social workers, and home health aides.
- Participate in case conferences, staff meetings, and ongoing education opportunities.
- Ensure adherence to agency policies and regulatory requirements throughout every stage of patient care.
- Daily travel within your assigned territory; mileage reimbursement provided.
Qualifications:
- Active RN license (in good standing) in the state of practice.
- At least 1 year of recent experience as a Home Health RN; field experience preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS required.
- Reliable transportation, a valid drivers license, and up-to-date auto insurance are essential.
- Strong assessment, organizational, and communication abilities.
- BSN and experience managing 2532 points per week are preferred.
- Knowledge of Medicare and home health compliance a plus.
