Seeking an experienced and compassionate Registered Nurse skilled in home health care for an engaging contract opportunity. This position supports patients in their homes, ensuring top-quality, patient-centered nursing within a collaborative and flexible field environment. The ideal candidate is self-motivated, organized, and comfortable providing care independently while maintaining high clinical standards and effective communication.
Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Responsibilities:
- Complete OASIS assessments and design individualized care plans.
- Deliver skilled nursing services (wound care, medication administration, IV therapy, chronic disease management).
- Monitor patient condition, communicate changes to the interdisciplinary team, and update physicians as needed.
- Offer patient and family education regarding disease management, treatment plans, and preventive care.
- Maintain thorough, accurate electronic records using Home Care Home Base (HCHB).
- Coordinate care with PTs, OTs, social workers, and home health aides.
- Attend case conferences and staff meetings; participate in ongoing education.
- Uphold all agency and regulatory standards for documentation and care delivery.
Qualifications:
- Active RN license (state of practice) in good standing.
- At least 1 year of recent home health experience (field preferred).
- Proficiency in Home Care Home Base (HCHB).
- Demonstrated OASIS experience.
- Strong assessment, critical thinking, communication, and organizational skills.
- Reliable transportation, valid drivers license, and current auto insurance.
- Ability to operate independently and manage a field-based schedule.
Preferred:
- BSN degree.
- Experience with 2532 points per week caseload.
- Knowledge of Medicare and home health compliance guidelines.
