Join a dedicated healthcare role delivering skilled nursing care directly to patients in their homes. This full-time, 13-week contract position offers the opportunity to apply your expertise as a Home Health Registered Nurse (RN) in a field-based setting serving patients within an assigned territory.
Location:
Based in Lawrence, MA, this position provides care within the local community. Lawrence is known for its historic mill buildings, vibrant cultural diversity, and close proximity to major urban centers. Nurses based here enjoy a balance of urban convenience and suburban accessibility.
Schedule:
- Monday Friday
- 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress; identify clinical changes and communicate promptly with the interdisciplinary care team.
- Educate patients and families on treatment plans, medications, and preventive health measures.
- Maintain timely, accurate documentation using Home Care Home Base (HCHB) electronic records.
- Collaborate with therapists, social workers, and home health aides to ensure coordinated patient care.
- Participate in case conferences, staff meetings, and ongoing professional education.
- Ensure compliance with regulatory requirements and agency standards.
Qualifications and Desired Experience:
- Current RN license in good standing for the state of practice.
- Minimum 1 year recent home health nursing experience (field experience preferred).
- Proficient with Home Care Home Base (HCHB) and experienced with OASIS assessments.
- Strong clinical assessment skills and critical thinking ability.
- Reliable transportation with valid drivers license and insurance.
- Ability to work independently and manage a flexible field-based schedule.
- Excellent communication, organizational, and time management skills.
- BSN preferred; familiarity with Medicare and home health compliance is a plus.
- Experience managing 2532 points per week preferred.
