An exciting opportunity is available for a Home Health Registered Nurse (RN) to provide compassionate, patient-focused care in a field-based setting. This full-time, 13-week contract position supports patients in their homes and focuses on promoting optimal health outcomes through skilled nursing care, patient education, and interdisciplinary collaboration.

Schedule & Setting:

• MondayFriday, 8:00 AM 5:00 PM
• 2532 points per week (average 35 patient visits per day)
• Field-based, travel to patient homes within assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions: wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, promptly communicate changes to care teams and physicians.
• Provide patient and family education on disease management and preventative care.
• Maintain accurate documentation within Home Care Home Base (HCHB).
• Coordinate with physical therapists, occupational therapists, medical social workers, and home health aides.
• Participate in case conferences, staff meetings, and ongoing education.
• Ensure compliance with regulatory and agency standards.

Required Qualifications:

• Active RN license in the applicable state, in good standing.
• At least 1 year of recent Home Health RN field experience.
• Proficient with Home Care Home Base (HCHB) and OASIS assessments.
• Strong skills in clinical assessment, communication, organization, and independent time management.
• Reliable transportation, valid drivers license, and auto insurance.

Preferred:

• Bachelors in Nursing (BSN)
• Experience managing a caseload of 2532 points per week.
• Knowledge of Medicare and home health compliance requirements.

$1,908 - $2,019/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
