Job Description

Deliver skilled nursing care directly to patients homes as a Home Health Registered Nurse (RN), offering compassionate, patient-centered services that promote health and recovery. This full-time, 13-week contract position requires an experienced RN who thrives in an independent, field-based environment while maintaining clinical excellence and effective communication.

Location: Laurel, MD
Laurel is a vibrant city nestled conveniently between Washington, D.C., and Baltimore, offering a mix of suburban comfort and urban amenities. With access to parks, cultural venues, and diverse dining options, it provides a welcoming community while serving as a hub for healthcare professionals supporting a variety of patient needs.

Schedule & Caseload:

  • Monday Friday, 8:00 AM 5:00 PM
  • Manage 2532 points weekly (approximately 35 patient visits daily)
  • Daily travel to patient homes within the assigned territory (mileage reimbursed)

Core Responsibilities:

  • Perform comprehensive OASIS assessments and develop individualized care plans
  • Deliver skilled nursing, including wound care, IV therapy, medication administration, and chronic disease management
  • Monitor patient progress and communicate any changes promptly to physicians and care teams
  • Educate patients and their families on health conditions, treatments, and preventive care
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB) system
  • Coordinate care with therapists, social workers, and home health aides
  • Participate in case conferences, staff meetings, and continuing education to uphold quality care standards

Qualifications:

  • Active RN license in good standing in the state of practice
  • Minimum 1 year of recent home health nursing experience with field experience preferred
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool required
  • Reliable transportation with valid drivers license and auto insurance
  • Strong clinical assessment, critical thinking, organizational, and communication skills
  • BSN preferred; knowledge of Medicare and home health compliance a plus
