Job Description
An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care in patients homes with a focus on quality, continuity, and patient-centered treatment. This role requires a self-motivated and organized professional comfortable working independently while maintaining clinical excellence and effective communication.
Key responsibilities include conducting comprehensive OASIS assessments, developing individualized care plans, and providing skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management. The RN will monitor patient progress, communicate changes to the care team and physicians, educate patients and families, maintain accurate electronic documentation in Home Care Home Base (HCHB), and collaborate with interdisciplinary team members to ensure coordinated care.
Qualifications:
- Active RN license in good standing
- Minimum 1 year recent home health experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB)
- Experience with OASIS assessments
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently with flexible scheduling
- Excellent communication and organizational skills
- BSN preferred
- Prior caseload management of 2532 points weekly preferred
- Knowledge of Medicare and home health compliance standards preferred
Job Details:
- Location: Laurel, MD area
- Contract: Full-time, 13-week duration
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (approximately 35 patient visits daily)
- Setting: Field-based, visiting patient homes
- Mileage reimbursement provided
