An opportunity is available for a Home Health Registered Nurse (RN) to deliver compassionate, skilled nursing care within patient homes in the Laurel, MD area. This full-time contract role spans 13 weeks and is ideal for an independent, organized nurse committed to clinical excellence and patient-focused care.
Responsibilities include conducting comprehensive OASIS assessments, developing individualized care plans, and performing skilled nursing tasks such as wound care, medication administration, IV therapy, and chronic disease management. The RN will closely monitor patient progress, communicate changes to healthcare teams and physicians, and educate patients and families on treatment plans and preventive care. Accurate documentation in Home Care Home Base (HCHB) and collaboration with therapists, social workers, and aides are essential duties.
Key Role Details:
- Contract Type: Full-time, 13-week contract
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points weekly (35 patient visits daily)
- Work Setting: Field-based within assigned territory
- Mileage reimbursement provided
Qualifications:
- Active RN license in good standing
- Minimum 1 year recent home health experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Excellent communication, time management, and organizational abilities
- Ability to work independently in a flexible, field-based schedule
Preferred Qualifications:
- BSN degree
- Experience managing 2532 points per week caseload
- Knowledge of Medicare and home health compliance standards
