An opportunity is available for a dedicated and experienced Registered Nurse specializing in home health care. This position is focused on providing expert clinical care to patients in their homes, facilitating recovery and promoting optimal health outcomes through professional, compassionate service.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Responsibilities:

• Conduct comprehensive patient assessments using OASIS and develop individualized care plans.
• Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management in patient homes.
• Evaluate patient condition, monitor progress, and maintain timely, accurate clinical documentation through Home Care Home Base (HCHB).
• Educate patients and families on treatment plans, medications, and preventive care.
• Collaborate closely with an interdisciplinary teamincluding therapists, medical social workers, and home health aidesto ensure seamless, patient-centered care.
• Participate in case conferences, staff meetings, and ongoing professional development.
• Comply with agency and regulatory guidelines for documentation and patient care.

Qualifications:

• Active, unencumbered RN license in the state of practice.
• At least 1 year of recent home health experience, with field-based care preferred.
• Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Reliable transportation, a valid drivers license, and current car insurance.
• Excellent assessment, communication, and organizational skills.
• BSN and experience managing caseloads of 2532 points per week are preferred.
• Thorough understanding of Medicare and home health compliance.
