A leading healthcare provider is seeking an experienced and compassionate Registered Nurse to deliver high-quality care to patients in their homes. This 13-week contract role focuses on providing comprehensive, patient-centered services within a flexible, field-based environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Field-based with daily travel to patient homes (mileage reimbursement provided)

Core Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing services, including medication administration, wound care, IV therapy, and chronic disease management
• Monitor patient status, adjusting care plans and communicating changes to interdisciplinary team members and physicians
• Provide education to patients and families on treatments, medications, and self-management strategies
• Document care using Home Care Home Base (HCHB) electronic system, maintaining accuracy and compliance
• Coordinate patient care with physical therapists, occupational therapists, social workers, and home health aides
• Participate in team meetings, case conferences, and ongoing training
• Uphold organizational and regulatory standards for documentation and patient care delivery

Qualifications:

• Active RN license in good standing within state of practice
• At least 1 year of recent field-based Home Health RN experience
• Proficiency with Home Care Home Base (HCHB) and OASIS
• Strong assessment, communication, and organizational abilities
• Reliable transportation, a valid drivers license, and current auto insurance
• Ability to work independently and effectively manage a mobile schedule
• BSN and experience managing 2532 points per week are preferred
• Familiarity with Medicare and home health compliance regulations a plus

$1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
