As an experienced Home Health Registered Nurse, you will deliver high-quality, patient-centered care in a field-based setting, working independently while ensuring continuity and excellence throughout your caseload.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
In this full-time, 13-week contract, you will:
- Manage a caseload of 2532 points per week (average 35 patient visits per day)
- Travel daily to patient homes within the assigned territory (mileage reimbursement provided)
- Use the Home Care Home Base (HCHB) electronic documentation system for efficient, accurate care records
- Conduct comprehensive OASIS assessments and craft individualized plans of care
- Deliver skilled nursing such as wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress, assess changes in condition, and collaborate promptly with care teams and physicians
- Educate patients and families on disease management, medication adherence, and preventive care
- Work closely with interdisciplinary professionals to coordinate patient-centered care
- Attend case conferences and meetings, and ensure regulatory and documentation standards are met
Qualifications:
- Active RN license in good standing
- Minimum 1 year of recent Home Health experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS
- Reliable transportation, valid drivers license, and auto insurance
- Strong assessment, time management, and communication skills
- Ability to work independently and manage a flexible field-based schedule Preferred: BSN, previous management of 2532 points per week, understanding of Medicare and home health compliance
