Job Description
Deliver skilled nursing care in patient homes as a Home Health Registered Nurse (RN) supporting individualized care plans and promoting patient well-being. This full-time, 13-week contract position involves independent field-based practice within an assigned territory, offering meaningful patient interaction and professional growth.
Location:
Based in Lancaster, PA, this role allows you to serve a diverse patient population within engaging community settings. Lancaster is known for its scenic countryside, rich history, and vibrant local culture providing a balanced lifestyle of work and recreation.
Schedule:
- Monday Friday
- 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Administer skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and evaluate patient progress, promptly communicating changes to the healthcare team and physicians.
- Educate patients and families on disease processes, treatment protocols, medications, and preventive care.
- Maintain accurate and timely documentation using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary team members such as physical therapists, occupational therapists, and medical social workers to coordinate care.
- Participate in case conferences, staff meetings, and continuing education.
- Ensure compliance with regulatory and agency standards.
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum 1 year of recent home health experience with field visits preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools required.
- Strong clinical assessment and critical thinking skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to independently manage a flexible, field-based schedule.
- Excellent communication, organization, and time management skills.
Preferred:
- BSN degree.
- Experience managing 2532 weekly points (35 patient visits per day).
- Knowledge of Medicare and home health compliance standards.
