Job Description
Take your nursing expertise into the community as a Home Health Registered Nurse, where youll deliver skilled care and empower patients to manage their health from the comfort of their own homes. This full-time contract position is ideal for an experienced RN seeking autonomy, flexibility, and the chance to make a difference through individualized care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract
- Daily field-based visits (typically 35 patient visits per day, totaling 2532 points per week)
Key Responsibilities:
- Perform in-depth OASIS assessments to develop and oversee personalized care plans
- Deliver skilled nursing such as wound care, IV therapy, medication administration, and chronic disease management
- Closely monitor patient progress, respond proactively to any changes in condition, and keep care teams/physicians well-informed
- Educate patients and families about diagnoses, care regimens, and preventive strategies
- Document all care and communication promptly within the Home Care Home Base (HCHB) system
- Collaborate with multidisciplinary teams including therapists, social workers, and home health aides for coordinated care
- Participate in case conferences, staff meetings, and professional development activities
- Uphold high standards for regulatory compliance and thorough documentation
Desired Qualifications:
- Current, active RN license (state of practice)
- At least one year of recent, hands-on home health experience
- Strong familiarity and proficiency with OASIS and Home Care Home Base (HCHB)
- Excellent clinical assessment, time-management, and independent decision-making skills
- Reliable transportation, drivers license, and proof of insurance
- BSN and experience handling Medicare compliance preferred
