Job Description
Deliver compassionate, skilled nursing care as a Home Health Registered Nurse (RN) providing patient-centered services in the comfort of patients homes. This full-time, 13-week contract role is ideally suited for a self-motivated nurse who excels in clinical assessment, independent work, and effective patient communication.
Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans tailored to patient needs.
- Provide skilled nursing procedures including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate updates promptly to physicians and care teams.
- Educate patients and families on diagnoses, treatment plans, medications, and preventive health principles.
- Document care accurately and timely using the Home Care Home Base (HCHB) electronic system.
- Collaborate with interdisciplinary professionals such as physical therapists, occupational therapists, medical social workers, and home health aides.
- Participate in case conferences, staff meetings, and continuing education, maintaining compliance with regulatory standards.
Qualifications:
- Active RN license in good standing within the state.
- Minimum one year of recent home health nursing experience; field experience highly preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools is required.
- Strong clinical assessment, critical thinking, time management, and organizational skills.
- Reliable transportation, valid drivers license, and auto insurance for patient home visits.
- Ability to work independently in a flexible, field-based schedule.
- Bachelor of Science in Nursing (BSN) preferred.
- Knowledge of Medicare and home health compliance standards advantageous.
- Experience managing a caseload of 2532 points per week preferred.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Caseload of 2532 points per week, typically 35 patient visits per day
- Field-based patient home visits with mileage reimbursement provided
