An excellent opportunity awaits an experienced Home Health Registered Nurse (RN) seeking a contract assignment with meaningful patient interaction and autonomy. This role delivers quality, patient-focused nursing care in clients homes, with support from an interdisciplinary care team.
Schedule/Hours:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (approximately 35 patient visits daily)
- Field-based, with daily travel (mileage reimbursed) within assigned territory
Key Responsibilities:
- Perform comprehensive OASIS assessments to guide individualized care planning
- Deliver skilled nursing services: wound care, medication administration, IV therapy, chronic disease management
- Monitor patient progress, identify changes in condition, and promptly communicate updates to physicians and the care team
- Educate patients and families on treatment plans, medication regimens, and preventive health
- Accurately document care using Home Care Home Base (HCHB)
- Collaborate with therapists, medical social workers, and home health aides for coordinated care delivery
- Participate in case conferences, staff meetings, and ongoing education
- Adhere to agency and regulatory standards
Qualifications:
- Active RN license in good standing for the state of practice
- Minimum 1 year recent Home Health RN experience, with field-based care preferred
- Proficiency in Home Care Home Base (HCHB) required
- Demonstrated experience with OASIS documentation
- Excellent clinical assessment, critical thinking, and communication skills
- Strong organizational and time management skills
- Reliable vehicle, valid drivers license, and current auto insurance required
- BSN preferred
- Familiarity with Medicare/home health compliance a plus
