An exciting opportunity is available for an experienced and compassionate Home Health Registered Nurse to provide high-quality care to patients in their homes. This position is ideal for someone who thrives on independent work, attention to detail, and ensuring patients receive the best possible care on their road to recovery.

Schedule:

• Full time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week, typically 35 patient visits daily
• Daily travel within an assigned territory (mileage reimbursement provided)

Responsibilities:

• Complete comprehensive OASIS assessments, develop and manage individualized patient care plans
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient condition, report clinical changes promptly, and document actions accurately using Home Care Home Base (HCHB)
• Educate patients and families on care tasks, disease management, medications, and preventive health
• Coordinate closely with physical and occupational therapists, social workers, and home health aides to ensure seamless, patient-focused care
• Participate actively in case conferences, staff meetings, and professional development activities
• Maintain compliance with all policies, documentation standards, and state/federal regulations

Required Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent field-based Home Health RN experience
• Proficiency with HCHB and strong familiarity with OASIS assessments
• Valid drivers license, reliable transportation, and current auto insurance
• Excellent assessment, communication, time management, and organizational abilities
• Strong ability to work autonomously and adapt to a flexible field-based schedule

Preferred:

• BSN degree
• Prior experience with a 2532 point weekly caseload
• Familiarity with Medicare and home health compliance

$1,839 - $1,946/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
