Job Description
Deliver skilled, compassionate nursing care in a dynamic home health setting as a Home Health RN. This position offers the opportunity to impact patient outcomes by providing individualized care in the comfort of patients homes. The role is best suited for a self-sufficient and organized nurse with strong clinical skills and a commitment to outstanding communication.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Daily travel within assigned territory (mileage reimbursement provided)
- Caseload expectation: 2532 points per week (typically 35 patient visits per day)
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop patient-specific care plans.
- Deliver skilled nursing services, such as wound care, IV therapy, chronic disease management, and medication administration.
- Monitor patient progress, assess changes in condition, and maintain close communication with the interdisciplinary team and physicians.
- Educate patients and their families about illnesses, treatments, medications, and preventive health practices.
- Ensure timely, accurate documentation using Home Care Home Base (HCHB).
- Collaborate effectively with therapists, social workers, and home health aides to optimize care.
- Participate in case conferences and ongoing professional development.
- Follow agency and regulatory standards for high-quality patient care.
Qualifications:
- Active RN license in good standing.
- At least one year of recent home health nursing experience (field work is preferred).
- Proficiency in Home Care Home Base (HCHB) and OASIS required.
- Strong clinical judgment, autonomous work habits, and reliable transportation with insurance.
- Excellent organizational, time management, and interpersonal skills.
- BSN and prior experience with similar caseloads preferred.
- Familiarity with Medicare and home health regulations is a plus.
