Deliver compassionate, expert care to patients in a home health setting utilizing your RN licensure and expertise. This is a contract role designed for nursing professionals who excel in delivering care plans, promoting patient independence, and fostering positive outcomes within patients homes.
Schedule/Hours:

Qualifications/Experience:
- Active and unencumbered RN license required
- At least 1 year of recent experience in a Home Health RN role
- Comprehensive knowledge of home care nursing principles and best practices
- Proficient assessment, triage, and documentation skills
- Valid drivers license and reliable transportation
- Ability to work independently and adapt care plans as needed
- Strong communication and organizational skills
Responsibilities:
- Conduct initial assessments and ongoing monitoring of home health patients
- Develop individualized care plans in collaboration with physicians and interdisciplinary teams
- Administer medications, perform wound care, and manage chronic disease or post-acute recovery
- Educate patients and families on treatment regimens, health maintenance, and self-care strategies
- Maintain accurate, timely, and compliant documentation of patient care and progress
- Identify changes in patient conditions, communicate with the care team, and update plans of care
