Elevate your nursing career by providing vital in-home care as a Home Health Registered Nurse. This full-time, 13-week contract demands expertise and compassion as you help patients maintain independence and achieve optimal health in their own homes. Bring your clinical skills and commitment to patient-centered care while collaborating with an interdisciplinary team and supporting both patients and families on their wellness journeys.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based, visiting patient homes daily within a designated territory (mileage reimbursement provided)
- Manage a caseload of 2532 points per week (approximately 35 patient visits daily)
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop customized care plans.
- Deliver skilled nursingincluding wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient status, document findings promptly in Home Care Home Base (HCHB), and update care teams as needed.
- Educate patients and families on illnesses, medications, and preventive health measures.
- Collaborate closely with physical therapists, occupational therapists, medical social workers, and home health aides to ensure seamless, coordinated care.
- Attend case conferences, staff meetings, and pursue ongoing clinical education.
- Ensure all care and documentation comply with regulatory and agency standards.
Required Qualifications:
- Active RN license (in good standing) in the state of practice
- Minimum 1 year of recent home health field experience
- Proficiency with Home Care Home Base (HCHB) and OASIS
- Reliable transportation, valid drivers license, and auto insurance
- Exceptional assessment, communication, and organizational skills
Preferred:
- BSN degree
- Experience with 2532 weekly points in a caseload
- Familiarity with Medicare and home health compliance
