Job Description
Deliver exceptional patient care as a Home Health Registered Nurse with a field-based position that offers meaningful opportunities to impact lives every day. This full-time, 13-week contract role focuses on delivering skilled nursing services in patients homes, ensuring continuity, high quality, and patient-centered care throughout the recovery process.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits per day)
Primary Responsibilities:
- Conduct OASIS-compliant assessments and create tailored care plans.
- Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and document patient progress, reporting changes to the interdisciplinary care team.
- Educate patients and families on treatment plans, medications, and disease prevention.
- Accurately complete documentation using Home Care Home Base (HCHB).
- Coordinate with physical/occupational therapists, social workers, and home health aides.
- Participate in case conferences, meetings, and ongoing education sessions.
- Travel daily to patient homes within the assigned territory (mileage reimbursement provided).
Qualifications:
- Active RN license in good standing (state of practice)
- At least 1 year of recent, field-based home health experience
- Solid working knowledge of Home Care Home Base (HCHB)
- Experience conducting OASIS assessments
- Strong clinical assessment, critical thinking, and organizational skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to manage a flexible, independent schedule
- Excellent communication skills
Preferred:
- BSN degree
- Experience managing a 2532 point weekly caseload
- Familiarity with Medicare and home health compliance
