Job Description
An experienced and compassionate Registered Nurse with a background in home health is sought for a rewarding, field-based opportunity specializing in patient-centered care. In this contract role, youll deliver skilled nursing services in patients homes, promote quality outcomes, and provide vital education to patients and families.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Key Responsibilities:
- Conduct comprehensive assessments using OASIS and develop individualized care plans.
- Provide skilled interventions such as wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient status, identify changes in condition, and promptly communicate updates to the care team and physicians.
- Educate patients and caregivers about disease processes, treatments, and preventive strategies.
- Maintain accurate, timely electronic documentation using Home Care Home Base (HCHB).
- Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for coordinated patient care.
- Attend case conferences, staff meetings, and continuing education sessions.
- Ensure compliance with all regulatory and agency standards for documentation and patient care.
Qualifications:
- Active RN license (state of practice) in good standing.
- At least 1 year of recent home health nursing experience (field work preferred).
- Proficiency with Home Care Home Base (HCHB) and OASIS required.
- Strong clinical assessment, organizational, and communication skills.
- Ability to work independently, manage a field-based schedule, and maintain reliable transportation.
- BSN and prior experience managing 2532 points per week preferred.
- Familiarity with Medicare and home health compliance standards is a plus.
