Job Description

Deliver compassionate patient care and clinical expertise as a Registered Nurse in home health, working directly with individuals in a field-based setting. This 13-week, full-time contract offers you the chance to make a meaningful impact by ensuring patients receive quality, personalized care where theyre most comfortabletheir own homes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized care plans for each patient.
  • Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, promptly identify any changes in condition, and communicate effectively with physicians and the care team.
  • Educate patients and families about disease processes, care plans, medications, and preventive measures.
  • Maintain accurate and timely documentation using Home Care Home Base (HCHB).
  • Collaborate closely with the broader healthcare teamphysical therapists, occupational therapists, social workers, and home health aidesensuring high standards and seamless communication.
  • Participate in case conferences, meetings, and continuing education opportunities.
  • Uphold all regulatory and documentation standards.

Qualifications:

  • Active, unencumbered RN license in the state of practice.
  • At least one year of recent, hands-on home health experience (field work preferred).
  • Proficient in Home Care Home Base (HCHB) and OASIS assessments.
  • Solid clinical assessment and critical thinking abilities.
  • Reliable transportation, valid drivers license, and current auto insurance required.
  • Flexible, self-motivated, and highly organized with excellent communication skills.
  • BSN and experience managing a caseload of 2532 points per week are preferred; strong knowledge of Medicare/home health compliance is a plus.
