A skilled Home Health Registered Nurse (RN) is needed to deliver compassionate, patient-centered nursing care in a home setting. This full-time contract role involves managing a caseload of 2532 points per week, typically seeing 35 patients daily. The position requires clinical expertise, excellent communication, and the ability to work independently while providing continuity and quality in patient care.
Responsibilities include conducting comprehensive OASIS assessments, developing individualized care plans, and administering skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management. The RN will collaborate with interdisciplinary teams, educate patients and families, and maintain precise documentation using the Home Care Home Base (HCHB) electronic system. Timely communication of patient progress and adherence to regulatory standards is essential.
Qualifications and Desired Experience:
- Active RN license in good standing
- Minimum of 1 year recent home health experience (field experience preferred)
- Proficiency in Home Care Home Base (HCHB) and OASIS required
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid driver's license, and auto insurance
- Ability to work independently with strong organizational skills
- BSN preferred
- Experience managing 2532 points per week and knowledge of Medicare/home health compliance preferred

- Monday Friday
- 8:00 AM 5:00 PM
