Experienced and compassionate Registered Nurses with strong home health backgrounds are invited to provide high-quality care in a patients own environment on a 13-week contract. This is a full-time, field-based opportunity with a supportive healthcare team, focused on empowering patients to achieve optimal health and independence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform OASIS assessments and create individualized, patient-centered care plans.
• Deliver skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease support.
• Evaluate and document patient progress, reporting any changes to the care team and physicians.
• Educate patients and families about disease management, medications, and preventive health.
• Maintain accurate, timely documentation in Home Care Home Base (HCHB).
• Collaborate with an interdisciplinary team of therapists, social workers, and aides.
• Participate in case conferences, staff meetings, and ongoing professional education.
• Ensure compliance with regulatory and agency standards for care delivery.

Qualifications:

• Active RN license (in good standing) in the state of practice.
• At least 1 year of recent field-based Home Health RN experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments required.
• Exceptional clinical assessment, organizational, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to manage a flexible field-based schedule and work independently.
• BSN preferred; prior experience with Medicare compliance and managing 2532 points per week is desirable.

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
