An opportunity is available for a dedicated and knowledgeable Registered Nurse with home health experience. In this contract-based position, youll play a pivotal role in delivering high-quality, patient-focused care in a field-based setting. Youll use your clinical expertise and independence to make a meaningful difference in the lives of individuals who rely on skilled nursing at home.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• Caseload: 2532 points per week, typically 35 patient visits daily

Key Responsibilities:

• Complete comprehensive OASIS assessments and design individualized care plans
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management
• Evaluate patient progress and promptly communicate any changes with the interdisciplinary team and physicians
• Educate patients and families on treatment plans, medication administration, and health maintenance
• Maintain precise documentation in Home Care Home Base (HCHB)
• Cooperate with therapists, medical social workers, and home health aides to ensure seamless care
• Participate in case conferences and ongoing training
• Uphold agency and regulatory standards in all aspects of care

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent home health nursing experience (field experience highly valued)
• Demonstrated proficiency with OASIS and HCHB electronic documentation
• Strong clinical assessment, time management, and communication abilities
• Reliable vehicle, valid drivers license, and current auto insurance

Preferred:

• BSN degree
• Experience managing a caseload of 2532 points per week
• Knowledge of Medicare and home health compliance

$1,845 - $1,952/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
