An exciting opportunity awaits an experienced and compassionate Registered Nurse with a passion for delivering high-quality home health care. This impactful position focuses on supporting patients in their homes, fostering independence, and ensuring optimal outcomes through personalized, patient-centered nursing care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract assignment
- Caseload: 2532 points per week (approximately 35 patient visits/day)
- Daily travel required within the assigned territory (mileage reimbursement provided)
Qualifications:
- Active RN license in good standing
- At least 1 year of recent home health nursing experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB) documentation system
- OASIS assessment expertise required
- Reliable transportation, valid drivers license, and current auto insurance
- Excellent clinical assessment abilities and critical thinking skills
- Outstanding communication, organizational, and time management skills
- Ability to work independently and adapt to a flexible field-based schedule
- BSN preferred
- Prior experience managing a 2532 point weekly caseload is a plus
- Knowledge of Medicare and home health compliance standards preferred
Key Responsibilities:
- Complete comprehensive OASIS patient assessments and develop individualized care plans
- Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
- Monitor and document patient status using HCHB, communicating changes to the interdisciplinary team
- Educate patients and families on disease processes, medications, and care plans
- Collaborate closely with therapists, social workers, and home health aides
- Participate in case conferences, meetings, and ongoing education as required
- Uphold agency, state, and federal standards for patient care and documentation
