Advance your nursing career by making a meaningful difference in the lives of patients in their own homes. We are hiring an experienced and compassionate Registered Nurse with a background in home health for a contract position delivering skilled care within a field-based setting.
Schedule and Job Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Field-based: travel required to patient homes within assigned territory (mileage reimbursement provided)
- Expected caseload: 2532 points per week (typically 35 patient visits daily)
- Use of Home Care Home Base (HCHB) for electronic documentation
- OASIS used for assessments
Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease management
- Monitor and evaluate patient progress, recognizing and reporting changes in condition promptly
- Educate patients and families regarding treatment plans, medications, and preventive care
- Document care accurately and promptly in HCHB
- Collaborate effectively with interdisciplinary care teams, including physical and occupational therapists, social workers, and home health aides
- Attend case conferences, staff meetings, and participate in ongoing professional development
- Uphold agency and regulatory standards for home health documentation and care delivery
Requirements:
- Active RN license (in good standing)
- At least one year of recent home health experience (field-based experience preferred)
- Proficient with Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical, assessment, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and organize a flexible field schedule
Preferred:
- Bachelors degree in nursing (BSN)
- Experience managing 2532 points/week
- Familiarity with Medicare and home health protocols
