Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation may be required
Role Overview: As a hospice RN, you will deliver high-quality clinical care and emotional support to patients in homes or residential settings. Youll serve as a cornerstone of the interdisciplinary team, ensuring patient comfort and effective symptom management while guiding families through the hospice journey.
Key Responsibilities:
- Conduct comprehensive assessments and develop tailored hospice care plans
- Provide direct nursing services focusing on comfort, pain management, and symptom control
- Administer medications and treatments per hospice standards and physician instructions
- Monitor clinical changes and relay updates to the care team and medical director
- Offer education to patients and families about disease progression and available support measures
- Collaborate with interdisciplinary team members, including social workers, chaplains, aides, and volunteers
- Maintain meticulous documentation in Home Care Home Base (HCHB)
- Attend case conferences, team meetings, and continuous improvement initiatives
- Ensure adherence to all hospice guidelines and regulatory standards
Qualifications:
- Active RN license (in good standing, state of practice)
- Minimum 1 year of recent hospice nursing experience
- Proficiency with Home Care Home Base (HCHB) documentation
- Strong clinical and assessment skills, with excellent communication
- Understanding of advanced pain management, end-of-life care principles, and family support
- Ability to work independently with reliable transportation and a valid drivers license
- Compassion, professionalism, and emotional resilience
- BSN and knowledge of Medicare hospice regulations preferred
