A Hospice Registered Nurse (RN) position is available, offering a full-time, 13-week contract to deliver compassionate, high-quality end-of-life care to patients in their homes or residential settings within the Kerman, CA area. This role focuses on enhancing patient comfort and dignity while supporting families through challenging times, requiring a clinical professional with hospice experience and strong interpersonal skills.
Key Responsibilities:
- Perform comprehensive nursing assessments and develop individualized hospice care plans.
- Deliver skilled nursing care, emphasizing symptom management, pain control, and comfort.
- Administer medications and treatments per physician orders and hospice protocols.
- Monitor patient condition changes and communicate updates to the medical director and care team.
- Educate patients, families, and caregivers on disease progression, comfort measures, and medication administration.
- Collaborate with interdisciplinary team members including social workers, chaplains, and aides to address holistic patient needs.
- Accurately document care and maintain compliance using Home Care Home Base (HCHB).
- Participate in case conferences, team meetings, and quality improvement initiatives.
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum one year recent hospice nursing experience.
- Proficiency with Home Care Home Base (HCHB) documentation.
- Strong clinical assessment, communication skills, and understanding of end-of-life care principles.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently managing a field-based schedule.
- Compassionate and emotionally resilient patient advocate.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience in palliative or end-of-life care.
- Knowledge of Medicare hospice regulations and documentation standards.
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM, with occasional weekends or on-call rotations.
- Approximately 45 patient visits daily.
- Daily travel within assigned service area; mileage reimbursement provided.
