An opportunity is available for an experienced and compassionate Home Health Registered Nurse to provide skilled care to patients in their homes. This contract position is ideal for a self-motivated RN who excels in autonomous environments and is committed to delivering patient-centered, clinically excellent care.

Schedule/Hours

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Responsibilities

• Complete comprehensive OASIS assessments and develop tailored care plans
• Deliver skilled nursing interventions such as wound care, IV therapy, and medication administration
• Monitor progress, recognize changes in patient condition, and promptly report to the healthcare team or physicians
• Provide patient and family education on disease processes, medications, care plans, and preventative health
• Document all care and communication accurately in Home Care Home Base (HCHB)
• Collaborate with interdisciplinary team members including therapists, social workers, and aides to ensure seamless care coordination
• Participate in case conferences and maintain regulatory compliance in documentation and care delivery
• Travel daily within the assigned territory; mileage reimbursement provided

Qualifications

• Active RN license in good standing in the state of practice
• Minimum one year of recent field-based Home Health RN experience
• Proficient with Home Care Home Base (HCHB) and OASIS assessment tools
• Strong skills in clinical assessment, communication, and organization
• Reliable transportation, valid drivers license, and auto insurance
• Ability to independently manage a flexible, field-based schedule
• BSN preferred; experience managing 2532 points per week and knowledge of Medicare/home health compliance are strong assets

$1,839 - $1,946/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
