Job Description
Deliver skilled, patient-centered nursing care as a Home Health Registered Nurse (RN) supporting individuals in the comfort of their own homes. This field-based contract role empowers you to deliver high-quality, compassionate care while working independently with the support of a collaborative team of healthcare professionals.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Daily travel within the assigned territory (mileage reimbursement provided)
- Caseload expectation of 2532 points per week (typically 35 patient visits per day)
Key Responsibilities:
- Conduct comprehensive OASIS assessments; create and implement individualized care plans.
- Provide skilled nursing interventions: wound care, IV therapy, medication administration, chronic disease management.
- Monitor patient progress, recognize changes in condition, and communicate with care teams and physicians.
- Educate patients and their families regarding diagnoses, treatment plans, medications, and preventive health practices.
- Document all care accurately and promptly in Home Care Home Base (HCHB).
- Coordinate with a full interdisciplinary team, including therapists and home health aides.
- Participate in care conferences, staff meetings, and ongoing education opportunities.
- Ensure all care delivery and documentation meets regulatory and agency standards.
Qualifications:
- Active RN license in good standing in the state of practice.
- Minimum 1 year of recent Home Health RN experience; field-based experience strongly preferred.
- Proficient in Home Care Home Base (HCHB) and OASIS.
- Strong clinical assessment, critical thinking, time management, and organizational skills.
- Valid drivers license, reliable transportation, and insurance.
- BSN and experience with 2532 point caseloads preferred.
- Knowledge of Medicare and home health compliance standards is a plus.
